Before we meet in person, I will send you an invitation via email to create your own client portal where you will fill out Intake Forms. They will all be filled out online and will be kept in your secure, encrypted, confidential electronic health record. The electronic system I use is called “Simple Practice.”
My Intake Forms include the following:
1. Informed Consent: This consists of 8 separate documents that require your electronic signature. We will layer in a review all of these documents during the assessment phase of family therapy, approximately 3-4 sessions.

c. Appointments
d. Communication
e. Payment
f. Court Policy
g. Mental Health Records
h. Social Media Policy
i. Good Faith Estimate (GFE)
2. Client Information Forms: This is a basic information form where you can also set your preferences for appointment reminders.
3. Intake Questionnaire: This is the initial family or individual assessment tool that you will begin filling out before your first session.
Before our first appointment, you will receive an email from me, S.AbigailMcCarrel no-reply@simplepractice.com. It will include detailed instructions for filling out the Intake Forms.
